The Hospice Clinical Liaison is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Hospice Clinical Liaison is responsible for assisting referral sources in identifying patients with care needs that match the services provided by hospice. Position Specific Responsibilities include attending and participating in staff meetings, educational programs, and community events; communicating identified needs and potential solutions to the Supervisor; explaining hospice services and benefits to patients, family members, and significant others; obtaining signatures on all required forms and providing copies to patient/responsible party; ensuring all forms and medical records are completed and turned in to the Admission Coordinator; establishing and maintaining significant professional relationships; working closely with community, hospital, and facility education representatives to meet growth and development targets; facilitating the admission process to and from hospital and long-term care-based referral sources; assessing referred patient’s clinical needs and disease status and conferring with physicians to establish eligibility; facilitating patient discharge from hospital to home; overseeing GIP patient admission process; providing education to hospitals and facilities on general inpatient care; and performing other duties as assigned. Education and/or Experience required includes an RN License, preferably a Bachelor's degree in Nursing, with 2-3 years of related experience, preferably in hospital and/or long-term care clinical settings and healthcare marketing. Skills required include mathematical skills, language skills for reading, writing, and presenting information effectively, and other abilities such as understanding and articulating hospice philosophy. Certification requires an active and unencumbered Registered Nurse license in state(s) of employment. Physical demands include standing, walking, manual dexterity, lifting up to 50 pounds, and exposure to bodily fluids and infectious diseases typical to healthcare settings.
